Transportation
Are you HIV positive? Do you need help with transportation to medical appointments or pharmacy visits? Rainbow Health can help.
For those who make less than 400 percent of the federal poverty guidelines (download the application form to the right to see if you qualify), we can arrange taxi transportation so you can access the medical care you need to stay healthy. We offer transportation services Monday through Friday from 8 am to 5 pm free of charge. All you need to do is fill out and return the forms below. Once your eligibility is verified, give us a call to request a ride at least two working days in advance of your appointment. You can schedule rides up to two months in advance.
We also have volunteer drivers who can assist with food shelf pick–ups and deliveries on a limited basis. We offer bus passes for eligible clients who cannot get a bus pass from other programs.
