Updating Your Birth Certificate with MDH

New as of January 2024!

The MN Department of Health now has a process to update a birth certificate with a new name or a change in sex designation (M, F or X).

To start this process, you can access the required form, which necessitates either of the following documents:

  1. Medical certification of appropriate clinical treatment for gender transition: This document must be an original letter issued by a licensed physician. The letter should identify the individual using the name and date of birth as stated on the current birth record. Additionally, the physician’s signature is required for validation.
  2. Certified copy of a court order: The court order must explicitly direct the amendment of the individual’s sex on the birth record.

It’s important to note that the completed form must be notarized by a public notary before submission.
