Clare Housing

Clare Housing operates 116 units of stable and affordable housing and has developed three resident-focused supportive housing options: Community-Based Care Homes, Supportive Housing, and a Scattered-Site Affordable Housing Program, for Minnesotans living with HIV/AIDS.

Community-Based Care Homes are staffed 24/7, each serving four residents who are living with HIV, significantly disabled and often need reliable care and support to live outside of a nursing home. Staff provides rehabilitation support, long-term care, and end-of-life care. The Supportive Housing program offers affordable housing and supportive services to low-income residents who have experienced homelessness or are at risk of homelessness at two supportive housing sites. Services offered include nursing care, medication administration, hands-on assistance with activities of daily living and independent living skill-building. Additionally, Clare Housing offers Project Cornerstone, a scattered-site affordable housing program with rental assistance and support to individuals and families who have experienced long-term homelessness.

Each program maintains a waiting list and submitting a formal application for each program is required. It is recommended that prospective residents work with an HIV case manager or social worker for assistance.
